Technically, it happens, but that is the least of one's problems.

No, they don't get it right every time, and it depends a lot on
conditions and subject. As Ansel Adams said, if you get one worthwhile
photo a month during a given year, it was a good crop. Of course, he was
referring to world-class fine art imagery.
