  To reverse-mount a lens. You screw the threaded end into the lens like a
filter and then mount the bayonet-ed end to the body. You will find that 
(within a very narrow range) you can focus at a much closer focusing 
distance.

  No, because they allow you to get even closer, higher magnification.
Plus they work with non-macro lenses to obtain higher mag. also.

  I've used ext tubes with 400mm lenses, but never with reflex lenses.

  That's up to you, depends on your needs or hardware lust. I find the 
OM  bellows to be rather big/clunky.
