Buddy,

there's no need to FOCUS on the gray card, just meter! If you want to make
it more precise, focus your long lens on the subject, then hold the gray
card in front of it (under the same lighting conditions as the subject) and
meter the blurred card. That's like a scrambled gray value. Good enough for
precise readings AND saves you the prothesis for your arm.
