There are several ways, depending on the availability of machine tools.  Someone
else has described a method using hand tools.  If you have access to a drill
dress, you can use an adjustable circle cutter set to cut the correct size hole.
Clamp the cap carefully to the table, be sure it is centered, adjust the drive
belts to the pulley set giving the slowest speed, and feed slowly.  If you
haven't used an adjustable circle cutter before, practice on some scrap
material.  They tend to grab, so the secret is a light touch on the feed lever.
When the hole is finished, smooth the burrs with fine sandpaper, working
tangentially so the scratches won't propagate as a crack.
