Without seeing whether or not you got a printable exposure on the film, here 
is my guess: The sequence of action is the mirror goes up and the diaphragm 
gets shut down by essentially the same mechanical action.  Once these are 
completed then the shutter can do its thing.  If the lens and/or 
teleconverter combination has too much internal friction to allow this 
sequence to complete, then you might get the odd behavior that you talk about.
