Perhaps I should have been more specific. General sorts of things like oily
aperture blades, fungus, and the like aren't the kinds of things I had
envisioned. Nor accident, which isn't a "failure" per se. More like:

Item: OM-PC
Problem: Mirror locks with new batteries
Sent for repair: Yes
Fixed (Shop): No
Fixed (User): No
Remarks: 'Circuit disease' Parts no longer available

OR

Item: 135/2.8
Problem: Aperture failed
Sent for repair: No
Fixed (Shop): No
Fixed (User): Yes
Remarks: Repaired spring retainer see: http://foo.bar.com

OR

Item: OM-10
Problem: Mirror Sticks
Sent for repair: Yes
Fixed (Shop): Yes
Fixed (User): No
Remarks: Full CLA for oily magnet fix see: blah blah blah

Make more sense?
