I use a Camcane, marketed by Billingham now, which has a variety of
support interfaces.  It is a carbon fibre device which doubles as a
walking stick and monopod.  It also has an elastic and leather strap
for carrying it over your shoulder.

It has a ball and cup system in which the cup screws into the bush on
the camera; a "stalker grip" which is a small curve into which your
lens fits nicely - normally a telephoto; and a "rubberneck" which
attaches to the top of the Camcane and screws into the bush providing
a flexible connection between the 2.  The last is the most useful for
odd positions, whereas the stalker grip can be used for portrait
orientation.

I carry my Camcane with me everywhere as a stick, but extend it as a
monopod when necessary.

 Is this just one of those things I'll have to keep practising and do
breathing exercises and such for?  Looking at the Leki hiking sticks people
are recommending, they seem like they'd be even more pivot-y at the base; at
least the one I'm using has a big rubber tip on it. (manfrotto 449-4, in
case it makes any difference -- it's the 4-section one, so it's not the most
solid, but it's definitely not flex in the monopod that's the issue here)
