Tiltall used to be the answer, but it is a shadow of former times.  I have one 
I bought in the 1970s, and it's still a rock.  Never did buy the camera though.

I looked at one once.  Brand new, something like US $3,000 for the whole kit I 
was looking at.  When I developed buyers' resistance, the salesman said that 
perhaps the price was too high for such as me to handle.  A semi-subtle insult 
to manhood; probably works a lot of times.  I replied that I didn't want a 
camera so expensive that I would have to spend all my energy guarding the 
camera, with no time left for photography.  Not the answer he was seeking.
