The software samples the picture preferably in a relatively featureless area like the sky. It essentially measures the pixel variation and makes a record of the pattern. It subtracts the variation pattern from the frame. You can adjust the amount of noise removal since too much may not look right. It is a repeatable and regular process, so you can put your camera's noise curve for each ISO setting into the software and you do not even have to sample anymore. Very cool. Some of them work with film grain in scanned images too. Neat Image and Grain Surgery seem to be the reigning champions of this software.
