The SPH (for spherical) is the dioptre correction you need for 
short-sightedness. CYL (for cylindrical) and AXIS are the amount and angle 
of correction for astigmatism.

Your optician should be able to grind these corrections for you, although 
for astigmatism you'll need to ensure the lens is mounted at the correct 
angle. For a rectangular lens this won't be a problem. Fo an eyecup taking 
circular lenses you'll need a mark on the lens.
