I tried 200/5 + bellows at modest extension a while ago.

Excellent working distance, but I was disappointed in the results,
perspective seemed strange in a way that I couldn't explain. So I
haven't pursued that further.

135/2.8 (ie the normal one) I found to give reasonably good results with
tubes or bellows. Also tried it a couple of nights ago with 65~116 and
T28 on the front -- gives nice dof but I need to do more work to figure out how
to use the flash properly in that setup.
