Matt,
Gallery is one possibility. http://gallery.menalto.com It requires php
and either netPBM or ImageMagick on your server. Once installed, it's
*almost* as simple as you mention. You can use a small application on
another PC to upload images to Gallery on your server (as well as other
methods) and it handles the rest. Probably has more features than you
want, but nothing says you have to use 'em all. Price is right, it's
free.
Another well-regarded app for this purpose is Coppermine.
http://coppermine.sourceforge.net/ It requires php, MySQL and either GD
or ImageMagick. Unfortunately I can't say more as I haven't used it.
