I have a Sunpak 422 and two OT-1D modules.  I just tested using one of 
my OM-2sp's.  One of the modules works as expected and gives both flash 
ready and blinking confirmation.  The other module also works as 
expected as long as I don't tighten the shoe down.  But, similar to what 
Jay reports below with his T20 connector, if I tighten it down I still 
get the flash ready indicator but the blinking confirmation doesn't work.

I can not see any physical difference between the OT-1D's except the 
serial number stamping.  The one that works fully in the 2sp is serial 
5F86C.  The other is serial number 7H89B.  The font used on the stamping 
of this one is slightly smaller than the the "OT-1D" label.  The font 
used on the stamping of serial number 5F86C is significantly larger than 
the "OT-1D" label.

Based on this I just decided to check my OM-2n as well.  Neither of the 
OT-1D's give blinking confirmation whether tightened down or not.  In 
fact, when tightened down, one of them (not sure which now) lost the 
main contact until I repositioned the flash.  Neither shoe ever lost the 
flash ready indicator, however.

Just to double check my own testing I tried a T-32 as well which, of 
course, operated perfectly.  It's hard to understand how there can be 
such variablity in these simple electrical contacts.  I just checked my 
personal archive and note that John Lind complained about this same 
problem a couple of years ago.
