Maybe the 2GB and 4GB cards have run into a lot of consumer resistance 
and the market just isn't big enough for big cards.

In my own case I have decided not to use cards larger than 2GB.  Even 
shooting raw on the 5D (12.7 MP) I get about 160 images on a 2GB card. 
It's not unusual to shoot 3-400 images at a long event so I typically 
use 2 and sometimes 3 cards.  It's a balance between avoiding card 
changes at possibly inopportune times and having too many eggs in one 
basket.  A friend has settled on 4GB cards for his 5D but I don't think 
he'll go larger than that.

I doubt that the owner of a typical consumer cam is under any pressure 
to go bigger than 1GB and the newer ones at 8-10 MP typically use 
something other than CF cards.  So, maybe the market for 2-4GB cards is 
pretty small and even smaller for 2-4GB CF cards.

I was reading in a (I think) a blog recently about someone who 
frequently swaps cards when convenient in order to use large cards but 
avoid the too many eggs in one basket problem.  I've been thinking about 
that but not sure whether frequent swapping might have some downside I 
haven't thought about... dropping and losing cards, damage to pins and 
contacts, static charges, etc. etc.
