Almost every case of this is caused by one of about 2000 different viruses
that exploit Microsoft Outlook. The virus will read the email addresses in
the inbox or in the contacts database. Then it replicates itself or sends
spam to everybody in the inbox or contacts database from one of the
addresses in the inbox or contact database.
Chances are, it did not originate from you, but from somebody that you've
emailed or are in their contact database. This kind of spoofing is extremely
common and probably accounts for up to 50% of all email messages transmitted
worldwide.
