That sounds like a 'Human Factor Facilitator's' explanation.  :-)
Did they happen to mention the fact that it just wasn't bloody unsinkable, as 
promised?
Engineering arrogance is a human factor too.
But harder to deal with when you're explaining the wonders of teamwork?
