I think it comes down to all the expenses involved in living and
working in a given location. While I save money living in a small
town, I make up for it by living in a bigger/nicer house than normal
and I commute 62 miles each way to work. Last year, while working in
Colorado, I had an apartment directly across the street from the
office. While the apartment was more expensive, not having any daily
commute cost offset that (and also gave me an hour a day of
non-commute time to work with).

Yes, living in NYC or SF is expensive if you try to have the same
standard of living as you would in Iowa. However, on the flip side,
the salaries for equivalent jobs are typically higher enough to make
up a lot of the difference.
