Typically, they took off with just enough fuel to guarantee liftoff and
getting to a decent flight altitude, then immediately docked with a tanker
to fill up.

I love the description in the Wikipedia page: "If a surface-to-air missile

<https://en.wikipedia.org/wiki/Missile_Approach_Warning>, the standard
evasive action was simply to accelerate and outfly the missile."
