I NEVER had that problem with a multitude of Olympus lenses -- including the 
aforementioned 16/3.5 -- but encountered it almost immediately when I got a 
third-party lens (and not even a cheap one: the Tamron 300/2.8). I sent the 
body to John for a new pin, sold the Tamron, and swore off third-party lenses.

Tamron made Adaptall adapters with four screws holding the mount on. Worked fine on all the OM 1 and 2 and double digit bodies. Then Oly added that little switch, and the adapters* didn't work. Tammy redesigned the adapters**, with a different label, so new/old could be recognized, but of course the old ones were still out there.

Sort of hard to fault them. They designed many Adaptalls, for many mounts, 
before Oly threw a curve ball.

I was forewarned here, used many third party lenses, including several Tamrons 
on OM-4(T(i))s, with nary a problem.
