If a screw head passes across the switch while the lens is twisted on
the bayonet mount, the edge of the screw hole cuts the top off the
switch.  It doesn't shear all of the switch off (which is a cone shape)
but enough to stop it working correctly.

It was the most common cause of failures of OM-4 cameras and even caused
Olympus to enclose bright pink warning labels with every OM-4 sold.
